Standard HLA matching has long been the foundation of transplant compatibility, and it remains central to the field. However, a complete match does not always predict a positive long-term outcome, which is why the science is expanding toward a complete view of each patient's immune profile.

A recent single-center study published in Pediatric Transplantation explores how MMDx tissue gene expression profiling, when combined with histopathology, may offer diagnostic insight in ambiguous pediatric kidney allograft biopsies.

The study found that molecular findings correlated with Banff histology and influenced clinical management in 19% of biopsies. Notably, among biopsies classified as borderline T cell-mediated rejection by histology, MMDx found no molecular evidence of rejection, and these patients maintained stable graft function without anti-rejection treatment during short-term follow-up.

Together, these findings suggest that combining molecular diagnostics with histopathology may offer additional context for ambiguous biopsy findings, helping inform clinical management and potentially limiting unnecessary treatment escalation.
